$95 - $150 charge (Some insurance carriers cover services
related to specific diagnosis codes; obtain referral from
your physician and check for coverage ahead of time with
CPFCC referral clerks available at each location)

30 minutes recommended for best results from initial visit.

Diabetes Education:
Diabetes Education: College Park Family Care Center is
proud to provide diabetes self-management training. This
gives people with diabetes the knowledge, skills and tools
they need to successfully manage their diabetes and avoid
many of the complications associated with the disease.
We have set up our diabetes education to be 3 hours worth
of education that is spread out over a 6-8 week time period.
Your diabetes education visits are one-on-one with a Certified
Diabetes Educator (or CDE eligible) dietitian. We feel
that our patient focused approach provides for the best
retention of knowledge, an individualized approach and
the repeated visits provide valuable feedback to you,
the patient, in
managing your diabetes.
Your First visit is 90 min. in duration. This
comprehensive appointment covers how to incorporate healthy
eating and exercise into your life, how to achieve healthy
weight control, an understanding on how your medications
work if you need them and learn how to monitor your blood
glucose to avoid the risks of complications.
2-4 weeks later: Your 2nd visit is 60 min. in duration.
This second visit will reinforce what you learned in your
initial visit with additional education on prevention
of heart disease and fat controlled eating. You will begin
learning to problem solve as you look at your blood sugar
patterns in relation to your possible medications, food
intake, activity level and emotions.
4 weeks later: Your 3rd visit is 30 min. in duration.
This is your final wrap up visit looking at blood sugar
patterns, weight control and exercise achievements. By
now your diabetes educator will have helped you create
a self-management plan that meets your individual needs.
*Some patients require additional visits if they need
training on insulin administration/medication adjustments
and ongoing support.
(Most HMO and PPO insurance companies cover diabetes
education very well. Just be sure that your CPFCC primary
care physician is covered by the insurance plan
that you carry and your diabetes education should be covered.
As always, it is best to check with your insurance company
prior to your fist visit to assure coverage.)
